When you file a complaint be prepared to provide the following information:
- Your name
- Your address
- Your phone number
- Brief description of housing problem
- Who discriminated against you (name, address, telephone number, company)
For HUD (under the Fair Housing Act) state that you were discriminated against because of your:

- Color
- Religion
- Gender / Sex
- Familial Status
- Disability
For CRD (under the Fair Employment and Housing Act) state that you were discriminated against because of your:
- Race
- Source of Income
- National Origin
- Marital Status
- Color
- Sexual Orientation
- Religion
- Gender Identity
- Sex
- Gender Expression
- Familial Status
- Genetic Information
- Disability
- Age
- Ancestry
- Arbitrary Characteristics
- Medical Condition
